Миеломная болезнь с секрецией моноклонального иммуноглобулина (Ig) А – вариант классической множественной миеломы, встречающийся у каждого пятого больного. Часто этот тип заболевания ассоциируется с нарушениями липидного обмена в виде гиперхолестеринемии, резистентной к гиполипидемической терапии.
В настоящей статье приводится описание клинического наблюдения пациента Б., 72 лет, у которого заболевание дебютировало с высоких цифр скорости оседания эритроцитов и гиперхолестеринемии. По данным иммунохимического исследования сыворотки крови определена моноклональная секреция IgA. Был поставлен диагноз: множественная миелома Аk, стадия IА, диффузно-очаговая форма. Гиперлипидемия. После трех курсов химиотерапии был достигнут положительный эффект: значительно снизился уровень IgА, показатели липидного обмена вернулись к нормальным значениям.
Multiple myeloma with monoclonal immunoglobulin (Ig) A exhibiting is a type of the classic multiple myeloma, occurring every fifth patient. This type of disorder is associated with hypercholesterolemia that is refractory to the conventional therapy with anti-hyperlipidemic agents.
This article presents a case of a patient B., 72 years old, whose disease debuted with high numbers of ESR and hypercholesterolemia. Ig Ak was identified by immunochemical analysis. Patient was diagnosed as having stage IA IgAk type multiple myeloma, diffuse lobular shape, with hyperlipidemia. Positive effect sustained after three courses of chemotherapy: level of IgA eventually subsided, and lipid metabolism returned to normal values.
