Аннотация
Синдром Мей-Тернера (СМТ) – компрессия левой общей подвздошной вены, повышающая давление в левой гонадной вене и потенциально провоцирующая варикоцеле. Несмотря на высокую распространенность варикоцеле и частые рецидивы после варикоцелэктомии, роль СМТ в его патогенезе остается недооцененной. В нарративном обзоре проанализировано 25 публикаций, в частности кейс-репорты, серии случаев, обсервационные и интервенционные исследования. Сводный анализ подтверждает, что у 67–90% мужчин с варикоцеле демонстрируется ≥25% компрессии левой подвздошной вены, а успешное стентирование приводит к регрессу симптомов варикоцеле и улучшению фертильности. Подчеркивается диагностическая ценность сочетания дуплексного ультразвукового исследования с компьютерной томографической/магнитно-резонансной венографией, а «золотым стандартом» остается венография с внутрисосудистым ультразвуковым исследованием, позволяющая одновременно измерить градиент давления. Обнаружены пробелы в данных о долгосрочной проходимости стентов и об эпидемиологии СМТ у бессимптомных мужчин. Предлагается включать оценку подвздошных сосудов в алгоритм обследования пациентов с рецидивирующим или билатеральным варикоцеле. Необходимо выполнение проспективных многоцентровых исследований, сравнивающих хирургическую и эндоваскулярную коррекцию.
