Аннотация
После внедрения новых опций лечения всегда важно увидеть, как результаты регистрационных исследований транслируются в рутинную клиническую практику. Клинические исследования помогают оценить эффективность, безопасность терапии и провести ее объективное сравнение с действующим стандартом лечения. Однако в них невозможно учесть все то многообразие ситуаций, с которыми онкологи сталкиваются в своей ежедневной работе. В настоящее время применение таргетной терапии при наличии определенных генетических нарушений, в частности драйверных мутаций в гене рецептора эпидермального фактора роста (EGFR), является неотъемлемой частью ведения пациентов с распространенным немелкоклеточным раком легкого (НМРЛ). Первые ингибиторы тирозинкиназы (ИТК) EGFR существенно изменили наши возможности в лечении этой группы пациентов, а сегодня мы уже используем ИТК EGFR III поколения. В обзоре проанализирован опыт применения ИТК EGFR III поколения осимертиниба через призму вопросов, которыми задаются практикующие врачи: оценка эффективности лечения в рутинной практике, ведение сложных категорий пациентов, в частности пациентов с метастазами в кости, головной мозг, пациентов с первично-множественными опухолями, тактика ведения пациентов с учетом пандемии COVID-19 и связанных с этим перерывов в лечении. Обзор литературных источников и международного опыта дополнен клиническими примерами из разных регионов Российской Федерации.
