Аннотация
Проведен обзор современных принципов диагностики и лечения некротического наружного отита, который является редким заболеванием, однако по мере прогрессирования и при отсутствии адекватного лечения имеет тенденцию к распространению до костей основания черепа, что может привести к летальному исходу. В группу риска входят пожилые пациенты с сахарным диабетом и пациенты с различными иммунными нарушениями. Рассмотрены основные инфекционные агенты в развитии некротического наружного отита, основным из которых является синегнойная палочка. Представлены обязательные и дополнительные диагностические критерии, которыми необходимо руководствоваться при постановке диагноза.
В лечебной тактике основное место занимает антибиотикотерапия. Препаратами выбора являются фторхинолоны и цефалоспорины III поколения, при этом ученые до настоящего времени обсуждают длительность терапии, которая чаще всего составляет 6 нед. Дискутабельным остается вопрос о хирургическом лечении, освещены показания и результаты данного метода у пациентов с некротическим наружным отитом.
