Введение. Подавление фактора роста эндотелия сосудов (VEGF) – обоснованный метод лечения рака почки, поэтому изучение комбинированной терапии бевацизумабом (моноклональное антитело к VEGF) и интерфероном-α (ИФН-α) являлось оправданным. Методы. В многоцентровом рандомизированном двойном слепом клиническом исследовании III фазы 649 пациентов, ранее не получавших лечение по поводу метастатического почечно-клеточного рака, были случайным образом распределены на две группы: в одной получали ИФН-α2a (9 млн МЕ подкожно 3 раза в нед) в комбинации с бевацизумабом (10 мг/кг 1 раз в 2 нед; n=327), в другой – плацебо в комбинации с ИФН-α2a (n=322). Основным оцениваемым показателем была общая выживаемость. Дополнительные цели включали оценку выживаемости без прогрессирования и безопасности. Представлены результаты окончательного анализа выживаемости без прогрессирования (ВБП). Анализ эффективности выполнялся по данным ITT-популяции. Результаты. 325 пациентов в группе бевацизумаба и 316 пациентов в группе плацебо получили минимум одну дозу исследуемого препарата. Медиана ВБП оказалась достоверно выше в группе бевацизумаба, чем в контрольной группе (10,2 мес по сравнению с 5,4 мес; HR 0,63, 95% CI 0,52–0,75; p=0,0001). Увеличение ВБП зарегистрировано у пациентов, лечившихся бевацизумабом независимо от группы риска и от того, изменялась ли доза ИФН-α или нет в ходе исследования. Наиболее часто регистрируемыми нежелательными явлениями 3 степени и выше были утомляемость [40 (12%) пациентов в группе, получавшей бевацизумаб, и 25 (8%) в контрольной группе] и астения [34 (10%) и 20 (7%)]. Выводы. Применение бевацизумаба в комбинации с ИФН-α в качестве терапии первой линии метастатического почечно-клеточного рака приводит к достоверному увеличению ВБП по сравнению с применением только ИФН-α.
