У больных, ранее не получавших лечение по поводу метастатического почечно-клеточного рака, выживаемость без прогрессирования (ВБП) увеличивалась при применении комбинации бевацизумаб + интерферон-α2a (ИФН-α2a) по сравнению с комбинацией ИФН-α2a + плацебо (исследование AVOREN). В данном исследовании выполнен анализ по подгруппам с целью определения эффекта от снижения дозы ИФН-α2a. Методы. Всего лечение ИФН-α2a по схеме 9 млн МЕ подкожно 3 раза в нед в комбинации с бевацизумабом в дозе 10 мг/кг или плацебо 1 раз в 2 нед вплоть до прогрессирования заболевания получили 649 больных. При развитии связанной с ИФН-α2a токсичности его дозу снижали до 6 или 3 млн МЕ. Анализ различий между группами лечения по таким показателям, как ВБП, частота объективного эффекта и переносимость лечения, проводили в подгруппе, в которой дозу ИФН-α2a редуцировали в ходе исследования. Результаты. В ходе исследования дозу ИФН-α2a пришлось снизить 131 больному из группы бевацизумаба и 97 больным из группы плацебо. Значение ВБП в группе бевацизумаба в комбинации со сниженной дозой ИФН-α2a было сопоставимо с соответствующими значениями для общей популяции (оценка Каплана–Мейера 0,524 против 0,427). Лечение бевацизумабом в комбинации со сниженной дозой ИФН-α2a хорошо переносилось больными, причем после снижения дозы ИФН-α2a наступало заметное уменьшение частоты нежелательных явлений. Выводы. Согласно результатам ретроспективного анализа по подгруппам у больных метастатическим почечно-клеточным раком, получающих лечение бевацизумабом в комбинации с ИФН, снижение дозы ИФН с целью уменьшения токсичности не влияет на эффективность лечения.
