Аннотация
Обоснование. Необходимость использования ингибиторов BRAF и MEK в лечении пациентов с метастатической и/или неоперабельной меланомой с мутацией в гене BRAF в определенных клинических ситуациях сегодня ни у кого не вызывает сомнений. Регистрация новой комбинации таргетных препаратов в России обусловливает необходимость представления более подробной информации медицинскому сообществу об ожидаемой эффективности и переносимости, а также возможных отличиях от существующих препаратов.
Цель. Продемонстрировать собственный опыт длительного использования нового поколения таргетной терапии – комбинации энкорафениба и биниметиниба – в лечении пациента с метастатической и/или неоперабельной меланомой с мутацией в гене BRAF, а также результаты регистрационного исследования III фазы COLUMBUS.
Материалы и методы. В данной статье представлен клинический случай лечения пациентки с диссеминированной меланомой с мутацией в гене BRAF, с множественными метастазами в печени, селезенке, лимфоузлах средостения и брюшной полости, желудке, костях с 2015 г. по настоящее время, с подробным описанием эффективности и переносимости, а также опубликованные данные по эффективности и переносимости комбинации энкорафениба и биниметиниба, полученные в ходе регистрационного исследования III фазы COLUMBUS.
Результаты. Показана высокая непосредственная и отдаленная эффективность, а также удовлетворительная переносимость комбинированной таргетной терапии энкорафенибом и биниметинибом как на примере представленного клинического случая, так и в ходе регистрационного клинического исследования. Собственный опыт и обновленные данные по выживаемости без прогрессирования и общей выживаемости в исследовании COLUMBUS подтвердили долгосрочную эффективность терапии КОМБО450 у пациентов с распространенной меланомой с мутацией BRAF V600.
Заключение. Новое поколение BRAFi и MEKi позволяет расширить возможности системной терапии пациентов с метастатической и/или неоперабельной меланомой с мутацией в гене BRAF, в том числе в подгруппе пациентов с множественными отдаленными метастазами в нескольких органах, включая желудок и кости.

Aim. To present of the study results and demonstration of our experience with the new generation of targeted therapy – encorafenib and binimetinib combination – in the treatment of patients with metastatic and/or inoperable BRAF+ melanoma.
Materials and methods. We present the clinical case of BRAF+ advanced melanoma patient with multiple metastases in the liver, spleen, mediastinal and abdominal lymph nodes, stomach and bones who is being treated with encorafenib and binimetinib since 2015 with the treatment efficacy and tolerability described in details, as well as the published data on the efficacy and tolerability of this combination from the pivotal phase III study COLUMBUS.
Results. High immediate and long-term efficacy, satisfactory tolerability of encorafenib and binimetinib combination are presented. Updated data on progression-free and overall survival in the COLUMBUS study confirmed the long-term efficacy of COMBO450 therapy in patients with advanced melanoma with BRAF V600 mutation.
Conclusion. New generation of BRAFi and MEKi combination expands options of systemic therapy for patients with metastatic and/or inoperable BRAF+ melanoma.
