Цель исследования – сравнить овариальный резерв у женщин с ожирением и здоровых женщин репродуктивного возраста. Материалы и методы. Обследовано 500 женщин молодого репродуктивного возраста (от 20 до 30 лет). Участницы разделены на равные группы (n=250) в зависимости от индекса массы тела (ИМТ): основная группа больных ожирением (ИМТ ≥30 кг/м2) и группа сравнения (ИМТ <30 кг/м2). На 2–3-й день менструального цикла определяли маркеры овариального резерва: сывороточные уровни антимюллерова гормона (АМГ), ингибина В, фолликулостимулирующего гормона, лютеинизирующего гормона, эстрадиола, тестостерона и прогестерона, а также на 5–7-й день менструального цикла ультразвуковые параметры – количество антральных фолликулов и объем яичников. Результаты и обсуждение. Статистически достоверно различались такие параметры овариального резерва, как уровень АМГ (2,7±0,47 нг/мл в основной группе против 3,8±0,63 нг/мл в группе контроля; p<0,05), тестостерона (1,4±0,3 нмоль/л в основной группе, 0,7±0,2 нмоль/л – в группе контроля; p<0,01), объем яичников (7,2±1,9 см3 в основной группе и 9,5±1,7 см3 – в группе контроля; p<0,05), а также количество антральных фолликулов (13,3±4,5 в основной группе и 20,7±7,2 – в группе сравнения; p<0,01). Заключение. Для больных ожирением женщин молодого репродуктивного возраста характерно снижение показателей овариального резерва по сравнению со здоровыми женщинами, однако значения оцениваемых параметров у больных ожирением остаются в пределах референсных значений.
Objective. To evaluate the ovarian reserve function in female patients with obesity in comparison with women without obesity. Materials and methods. This study evaluated 500 caucasian women, age 20–30 years, 250 with obesity (body mass index, BMI ≥30 kg/m2) and 250 without obesity (BMI <30 kg/m2). Anthropometrics, serum concentrations of anti-Mullerian hormone (AMH), inhibin B, follicle stimulating hormone, luteinizing hormone, estradiol, progesterone, and testosterone were compared as ovarian volume (cm3) and antral follicle count (AFC), determined by ovarian transvaginal ultrasonography. Results and discussion. We reveal statistically significant difference in following parameters: AMH (2.7±0.47 ng/ml vs. 3.8±0.63 ng/ml; p<0.05), testosterone (1.4±0.3 nmol/l vs. 0.7±0.2 nmol/l; p<0.01), ovarian volume (7.2±1.9 cm3 vs 9.5±1.7 cm3; p<0.05), and AFC (13.3±4.5 vs 20.7±7.2; p<0.01) in obesity group vs. control group respectively. Conclusion. Ovarian reserve function is significantly lower in obese patients than in healthy control subjects of young reproductive age, but ovarian reserve parameters are in normal reference range even in obese patients.
