Цель – оценка частоты и факторов риска послеоперационной фибрилляции предсердий (пФП) у больных хронической ишемической болезнью сердца (ИБС) с острым почечным повреждением (ОПП), развившимся в связи с коронарным шунтированием (КШ). Материалы и методы. Обследовано 90 больных, перенесших КШ, в возрасте 58±7 лет с длительностью ИБС 6±6 лет. Ранее перенесли инфаркт миокарда 80% пациентов. Хроническая сердечная недостаточность в рамках II функционального класса (ФК) выявлена у 53,3%, III ФК – у 46,7% больных. Многососудистое коронарное поражение имели 75,6% пациентов. КШ на работающем сердце выполнено 11,1%, в условиях искусственного кровообращения – 88,9% больных. Уровень креатинина в крови определяли по методу Jaffe, cкорость клубочковой фильтрации (СКФ) рассчитывали по формуле СКD-EPI. ОПП диагностировали с учетом критериев KDIGO 2012 г. Результаты и обсуждение. Частота транзиторного ОПП после КШ составила 33,3%, пФП – 17,8%, пФП среди лиц с ОПП – 20%. Развитие ОПП ассоциировано с более высокими уровнями тропонина Т после КШ (медиана [25-й; 75-й перцентили] – 0,36 [0,24; 0,99] против 0,28 [0,11; 0,50] нг/мл; р=0,037), более частым применением инотропных средств (60 и 25%; р=0,002), большей длительностью инотропной терапии (2,0 [1,0; 2,5] против 1,0 [0; 1,0] дней; р=0,001). Доля больных, имевших пФП, среди лиц с ОПП и без такового существенно не различалась (20 и 16,7%; р=0,7). Снижение СКФ <39 [29,8; 45,7] мл/мин/1,73 м2 после КШ – наиболее значимый фактор риска пФП у больных ИБС с ОПП.
